(1) Microsoft requires organizations to have at least 85 percent of assigned donated (free, granted) licenses to be actively used. "Assigned licenses" means licenses that have been assigned to a specific user within your organization. "Active use" means that those individuals have used at least one Microsoft cloud service in the last 90 days. This includes those who are grandfathered into a donated or free-version Office 365 E1 or E2 licenses.
(2) To make sure your subscription is current, you will need to connect to the Internet at least once every 30 days. You will also need to be connected to the Internet to do real-time collaboration or to save items to the cloud.

"On-premises" refers to the traditional method of paying for software. You pay for a license once, and you can use the software for as long as you want, in accordance with the license agreement.
Microsoft cloud subscriptions are the subscription-based versions of Microsoft's offerings. You renew these on a yearly basis, and you may continue using them for as long as you subscribe.
Not necessarily. While some of Microsoft's cloud services — like OneDrive or Microsoft Teams — require an Internet connection to use to their fullest capacity, many of Microsoft's cloud-enabled products work well offline.
